Try this first
- Take an immediate backup of files plus database
- Identify hacked URLs via Search Console coverage report
- Remove spam content from DB (wp_posts) and suspicious files in plugins/themes
- If clean state is unclear, restore a pre-hack backup and migrate changes manually
